New Client

Seed of Hope Counseling Services offers downloadable copies of our new client forms. These forms may be downloaded and printed at your convenience to bring with you on your first visit to our office. We are unable to accept these forms electronically or via fax due to privacy laws.
When arriving for your first appointment it is suggested you arrive 5-10 min. early to complete any additional paperwork that maybe required by your insurance company or employer.

Seed of Hope Counseling Services normal business hours are Monday - Friday 10am - 6pm. (Saturday available by appointment only) No calls are accessed or returned on weekends, holidays or when the office is closed for any reason.

Please note, Seed of Hope Counseling does NOT provide any PHONE, EMAIL or INTERNET counseling. It has been our experience that face-to-face sessions facilitates a greater depth of understanding and are more productive. We are aware that there may be times when limited phone contact is necessary for business matters; however, any lengthy calls will be covered by the client.
